what is child abuse/neglect?
- physical, psycholoical or sexual injury to a child
- if physical than resulting injuries (brusies, fractures, burns, bites, welts)
- psychological abuse may be evidenced in actions of the child
- sexual abuse may be discovered incidentally and investigated when the child complains of discomfort in the genital region or if the child reports the abuse
- neglect also may be considered a form of child abuse
What is shaken infant (baby) syndrome?
- SIS or SBS
- injuries incurred by infant or toddler who has been shaken forcibly enough to cause intracerebral bleeding (brain continually striking the inside of the cranial vault and then recoiling against the other side of the skull = tiny vessels rupture), resulting in a closed head injury
- baby experiences altered levels of consciosuness to complete loss of consciousness, irritabiltiy, changes in skin color (paleness or cyanosis), vomiting, lethargy, convu,lsion, no outward indiciatin of physical trauma, fractures, dislocated bones and neck or spinakl injuires, retinal hemorrhage
diagnose and treat SIS/SBS?
diagnose - subdural hematoma, cerebral edema, retinal hemorrhage, CT for brain hemorrhages
treat - control bleeding and cerebral edema, reduce ICP, damage is permanent, even fatal, may have visual deficits, blindness, IDD, cerebral palsy
What is elder abuse/neglect
- different forms: physical abuse, self-neglect, verbal abuse, psychological abuse, abandonment, violation of personal rights, financial abuse, withholding actions that result in harm or threatening harm to health or welfare
- signs r brusing, fractures, malnutriton, decubitus, poor hygeine, alopecia, poor general heatlh status
What is psychological or verbal abuse?
- intentional and systemic diminishment of another’s self-worth or self-esteem
- often leads to self-destructive behavoir (anorexia, bulimia, obesity, alcholoism, self-mutilation, drug addiction, depression, suicide), emotional guarding, lack of self-esteen and self-worth and often r unable to bond with others
diagnose and treat psychological or verbal abuse?
diagnose - difficult to diagnose bc no physical contact, usually made in private, absued may feel responsible (defend it)
treat - best is prevention, buiding of self-esteem and self-worth
What is intiamte partner violence?
- threatened or actual physical abuse, pattern of behavior in which the victim is repeatedly physically assaulted by her or his partner
- aka domestic violence, spouse abuse, battered spouse syndrome
- mostly women, bruising in area tha tis conceal by clothing, avoid making eye contact
- diagnose by physical injuries, assess emotional state
- treat physical injuries then discuss sitation, counseling, listening
What is sexual abuse?
- nonconsensual sexual contact or activity
- may be fondling of genitals or breasts, penetration of vagina or rectum w/ fingers or other objects, forcing the victim to touch the perpetrator on or around the sex ogans, exposing child to porn or taking pornographic photos of child, itching or burning in urethral and vaginal areas and exhibit symtpoms of a UTI, if children then unusual obsession with genital region and frequently touch themselves abnormally
What is rape/ sexual assault?
- intercourse forced on a person against her or his will
- vary with degree of violence, some report having pain in the pelvic and perineal regions or being choked or restrained, threats of death, brusing, torn clothing, dirt and debris ground into clothing or hair, disheveled appearance, withdrawal, anxiousness, avoidance of eye contact, bruising, tears, lacs, around or on genital, rectual or mouth, tenderness, semen on inner aspect of thighs, genital areas, mouth, nares or pubic, sodomy if brusing or lac in anal area
diagnose and treat rape
diagnsoe - findings from history, clinical exam, physical exam, pelvic exam, sex crim evidence gathering, samples of debris, clothing, saliva, hiar, pubic hair, fingernails, scraping, urinalysis, blood tests for STDS, AIDS< pregancy, photos
treat - counseling, prophylaxis for pregnancy, reinforcement that the victim is a survivor of a violent crime and is not responsible for the incident,
