What is the difference between intravascular and extravascular?
Intravascular: Complement Mediated rupture of sensitized RBC
Extravascular: Macrophage ingest sensitized RBC
Describe the pathophysiology of Intravascular Haemolytic Transfusion Reaction
Antigen and Antibody complexes activate complement cascade and with high IgM and IgG titre
Haemolysed RBC
What antibodies commonly cause instravascular reaction?
ABO antibodies, anti-P, PI, Pk, anti-Vel
What are the symptoms symptoms of Intravascular reactions?
- Fever and chills
- Burning sensation at site of infusion
- Chest restriction, tachycardia
- Hypotension , shoc,
- Lower back pain, nausea, vomiting
- Feeling of impending doom
- Hemoglobinuria
- Bleeding due to DIC
What are the major complications of Intravsacular reactions
- Renal failure
- Disseminated Intravascukar Coagulation
- Fatal in 10% of ABO incompatibilities
Define Disseminated Intravascular Coagulation
Inappropriate excessive and uncontrolled activation of haemostatic process
How to manage Intravscular Reactions
- Stop transfusion
- Check recipienr identity against blood bag
- Maintain blood pressure and treat shock with IV saline
- Diuretic
- Transfusion Reaction investigation
Describe the pathophysiology of Extravascular reactions
RBCs sensitized with IgG and/or C3
Antigen Antibody complex forms
Macrophages phagocytoses sensitized RBCs
List symptoms of Extravascular reactions
- Fever and chills
- Bilirubin in plasma and urine
- Not fatal
What are the complications of Extravascular reactions
- Anemia
2. Jaundice
What antibodies are associated with Extravascular reactions
IgG antibodies of: Rh system Kell system Duffy system Kidd system
Describe the pathophysiology of delayed haemolytic transfusion Reaction
Patient has Antibody at undetectable level
Antibody Screening is negative
Compatible blood
Patient has secondary immune response to an antigen from transfusion
List the symptoms of Delayed reaction
- fever and chills.
- Bilirubin in urine and plasma
- Occurs 24 hours to 28 days later

- C, c, E

- Pre transfusion sample
