1) Stay with the patient and discontinue the transfusion IMMEDIATELY
2) Remove blood and blood tubing according to your agencies policy
3) Start Normal Saline with new primed tubing at keep vein open rate
4) Document Vital Signs and notify PHCP
5. ) Notify Blood bank of transfusion reaction and monitor client for anaphylaxis
