Traction Flashcards
1
Q
Bone effects
A
- Wolf’s law: respondes to compression/distraction doesnt correlate
- No direct effect on bone EXCEPT can reverse immob weakness
2
Q
Ligament effects
A
- Slow loading: absorbs forces/lengthens
- Encourages adaptive changes in length/strength
- Activates proprioceptive fibers: gating mechanism to override pain sensation
- Applies pressure on structures outside of lig
3
Q
Disc effects
A
- Decrease central pressure
- Return nucleus to center
- Tension annulus/ligs around disc
- Move structures relives nerve/vasc pain
- Improve fluid exchange in canal/disc
- Reduce disc herniation
4
Q
Facet effects
A
- Seperates 1-2mm: space for meniscoid, synov capsule, bone fragments
- Decrompress articular cartilage
- Increase synovial exchange to cartilage
- Decrease degen changes of OA
- Stimulate proprioceptive nerves at facet: gate effect
5
Q
Muscle effects
A
- Stretch/relax: less than 10 mins
- Better blood flow
- Activates proprioceptors
- Inhibit reflex guarding
- Decrease muscle irritation
6
Q
Nerve effects
A
- Decrease pressure on nerve
- Increase blood flow to nerve
- Decrease edema
7
Q
Contraindications
A
Acute strain/sprain Acute inflam Fractures Vert joint instabil Tumors Bone diseases Osteoperosis Infections Vascular conditions Pregnancy Cardiac/pulm problems
8
Q
Lumbar Traction Parameters
A
-Position:
Neutral= IVF opening
Prone= disc herniation, other modalities
Supine=posterior seperation, stenosis
-Force:
min- 1/4th body weight
50% body weight=vert seperation
Start at 30; increase from 65-200
-Sustained vs. Intermit
Sustained= disc
Intermit=proprioceptive fibers
short holds 10s=stretch ligs/muscle
-Duration
disc herniation=8-10min
decrease muscle activation=
9
Q
Cervical Traction Parameters
A
-Position
flexed 20-30=IVF
neutral=OA/AA
-Force
20lbs start
up to 50 as tolerated
-NO SUSTAINED
intermit at least 7s holds
same as lumbar
-Duration
8-10min; up to 20-25
