topical therapy MS Flashcards
Wich products have keratolytic and keratoplastic properties?
- Tar
- Selenium sulphide
- Salicylic acid
- Sulphur
Action of sulphur?
- keratoplastic
- keratolytic
- antibacterial
- antifungal
- antiparasitic
- antipruritic
MOA of sulphur?
- keratoplastic: production of cystine;
- keratolytic: H2S breaks down keratin;
- antibacterial-antifungal-antiparasitic: due to conversion to pentathionic acid and H2S by cutaneous bacteria and by KCs
Action of salicylic acid
- keratoplastic (0.1-2%)
- keratolytic (3-6%)
- antibacterial (static)
- antipruritic (mild)
MOA of salicylic acid
- keratolytic: by decreasing pH increases keratin hydration and solubilises intercellular cement
Actions of tar
- keratolytic
- keratoplastic
- degreasing (mild)
- antipruritic (anesthetize peripheral nerves)
- anti-inflammatory (vasoconstriction)
MOA of tar
keratoplastic: reduced DNA synthesis in basal cells
Actions of benzoyl peroxide
- keratolytic
- degreasing
- follicular flushing
- antibacterial (residual effect x 48h)
- antipruritic
MOA of benzoyl peroxide
keratolytic: metabolized to benzoic acid that lyses intercellular substance of horny layer
Actions of selenium sulphide
- keratolytic
- keratoplastic
- degreasing
- anti-fungal (Malassezia)
MOA seleenium sulphide
keratolytic: interference with hydrogen bond formation
Actions of Zn gluconate
- antiseborrheic,
- antibacterial
Actions of ammonium lactate
- keratolytic,
- keratoplastic
What do emollients do
decrease TEWL
Name occlusive emollients
- Vegetable oils: olive, cottonseed, corn, peanut, safflower, sesame
- Animal oils: lard, whale oil, anhydrous lanolin, lanolin with water
- Hydrocarbons: paraffin, petrolatum (=mineral oil)
- Waxes: white wax, yellow wax, spermaceti
- Silicones
What are humectants or moisturizers
hydroscopic agents incorporated into SC
Name humectants (moisturisers)
- Propylene glycol, glycerine (demulcents)
- Colloidal oatmeal
- Urea (2-20%)
- Sodium lactate
- Carboxylic acid, lactic acid
MOA of urea (2-20%)
-at higher concentration is keratolytic (solubilisation of prekeratin and keratin, breakage of hydrogen bonds, dissolving intercellular substance)
Name topical retinoids
- Tretinoin
- Adapalene (less irritating than tretinoin)
- Tazarotene (min systemic absorption)
Topical retinoids-Tretinoin (vitamin A acid, retinoic acid)
name indications
- acne (D, C)
- localized keratinisation disorders
Topical retinoids-Tretinoin (vitamin A acid, retinoic acid)
MOA
- increases epidermal turnover time,
- reduces cohesiveness of KCs
- normalized maturation of follicular epithelium
- comedolytic
Topical retinoids-Tretinoin (vitamin A acid, retinoic acid)
side effects
irritations (cats )
Adapalene- additional activity
- inhibition of neutrophil activation,
- inhibition of lipoxygenases
