Mifepristone = anti progesterone given under supervision
then
Misoprostol = PGE1 analogue given 36-48 hrs later.

Contraindication of Medical TOP
A
Hypersensitivity or allergy to prostaglandins or product,
Ectopic pregnancy
Gestational trophoblastic disease
Intrauterine device remove
Obstructive cervical lesions e.g. fibroids
High suspicion of placenta accrete or
High risk of uterine rupture.

Surgical TOP
A
6-14wk Day surgery D&C Misoprostol given the night before ABX Contraception Anti D - 220 Units.

Risk of surgical TOP
A
Haemorrhage Trauma Sepsis mortality 1:100 000 Normal Cx of surgery Psychological Rarely - Cervical stenosis, incompetence, Asherman's syndrome.

Serious CX
A
Excess bleeding and cramps and need for a 2rd D&C
Infection, fever chills, unwellness, offensive D/C or raised WCC
Incompleteness,

Follow up
A
Approx 14 days later Check for infection, bleeding diminishing, emotional well BHCG Test Contraception Blood loss can last up to 18 days.
