Tocolytics Flashcards
Tocolytics are give to
Delay delivery up to 7 days or after 20 weeks
What four drugs are given to delay contractions/labor?
Indomethacin
Magnesium Sulfate
Nifedipine
Terbutaline
Indomethacin is also known as
Cyclo-oxygenase Inhibitor
What is the MOA of Indomethacin?
Non specific inhibition of COX1 & 2 enzymes
Decreases the production of prostaglandins (IP3), which then inhibits Ca+ release
What are the side effects of Indomethacin?
Gastritis
Nausea
Premature PDS closure
Oligohydramnios (decrease in amniotic fluid)
What is the MOA of Nifedipine, a CCB?
Blocks the calcium channels
Inhibits the release of intracellular Ca+ from the sarcoplasmic reticulum
What are the side effects of Nifedipine?
HA
Dizzy
Vasodilation
Palpitations
What is the MOA of Terbutaline, a Beta Agonist?
Binds Beta 2 adrenergic receptors
Stimulates adenyl cyclase
Increases the formation of cAMP
Inhibits MLCK
What are the side effects of Terbutaline, a Beta Agonists?
Vasodilation
HOTN
Tachy & palpitations
Tremor, SOB
Pulm edema
Fetal tachy/hypoglycemia
Atosiban, an oxytocin receptor antagonists has this MOA
Competitive inhibition of oxytocin, binding to oxytocin receptors
Prevents IP3 formation & Ca+ release from SR
What are the side effects of Atosiban, an oxytocin receptor antagonist?
Hypersensitivity reaction
Injection site reactions
What is the MOA of Nitric Oxide donors, like nitroglycerin
L-arg–>NOS–>NO= Guanyl cyclase= increased formation of cyclic GMP= which then activates MLCP, causing smooth muscle relaxation
What are the side effects of taking Nitric Oxide Donors?
HOTN
HA
Flushing
Palpitations
What is the MOA of Mg+?
Unknown, but could be related to:
Competes w/Ca+
Membrane hyper-polarization
Inhibition of MLCK & decreases contractility
What are the side effects of Mg+?
HA
Diaphoresis
Flushing
