TMJ Flashcards
Pain Complaint of Pt
facial pain, jaw discomfort, ear discomfort
Hard crepitus
walking on gravel
arthritic changes
Soft crepitus
rubbing knuckles
symptomless
Early Clicking
developing
Late Clicking
chronic
Disc Displacement with Reduction
closing
Reciprocal Clicking
opening and closing
Causes of Clicking
adhesions: bruxism
uncoordinated muscle action: lateral pterygoids
soft or popping clicks: ligament movement/ soft tissues behind condyle
hard or cracking clicks: joint pathology
Lock Jaw
restrictions upon opening and closing
Closed Lock
mouth does not fully open
d/t anterior displacement of disc, inert tissue tightness, muscle spasm
athletes hit by a ball/ opponent’s limb
Open Law
mouth does not fully close
two clicks felt on the disc
>posterior rim
>anterior rim
Ruling out Cervical
vestibular systems
vertebrobasilar artery
cerebral spine affectation
muscle imbalance from spinal malalignment
Functional Limitations
eating (soft, hard), biting, laughing, smiling, yawning, talking, shouting, chewing on L and R/ both
Partial Edentulism
loss of one or more teeth causing others to shift to fill the space leading to malalignment
Bruxism
grinding/ clenching of teeth especially when sleeping
headaches in the morning
hypertrophy
facial, jaw, or tooth pain
Periodontist
specializes in the study of tissues around the teeth and diseases
Orthodontist
correction or prevention of irregularities of teeth (retainers, invisaligns, etc.)
Endodontist
treatment of diseases of tooth pop, root canal, and perapical areas
Review of Systems
hearing loss
ear ache
dizziness
ringing
blocking
inner ear, cervical spine, vestibular problems
Related Psychosocial Issues
anxiety or depression
drug abuse
failure of conventional therapies
secondary pain
major life events causing stress
stress due to pain greater than 6 months
stress related disorders
inconsistency in response to drugs
inconsistent, inappropriate, and vague reports of pain
overdramatization
symptoms that vary with life events
Underbite
mandibular teeth in buccoversion (lie anterior to maxilla)
Overbite
maxillary teeth extend to anterior mandibular teeth
Normal Dental Occlusion
small overbite (1-2 mm); most common position
Overjet
distance between maxillary incisors to mandibular incisors when the jaw is closed (2-3 mm)
Occlusal Interference
premature contact of teeth; deflect lateral/ anterior
Crossbite
mandible are lateral to upper teeth of one side and medial to the opposite side
Malocclusion
any deviation of normal occlusion
Class I Malocclusion
slight modifications
normal AP
Class II Malocclusion
overbite
mandibular teeth it posterior
Class II Division 1 Malocclusion
significant overjet of maxillary incisors
large/ horizontal overlap
Class II Division 2 Malocclusion
non significant overjet of maxillary incisors (lateral flaring)
deep/ vertical overlap
Class III Malocclusion
underbite
mandibular teeth are anterior
Orthognathic Profile
normal straight jawed
Retrognathic Profile
chin behind the vertical line
receeding chin
Prognathic Profile
chine in front of the vertical line
protruded chin
Tongue Movement
limited by lingual frenulum
Tongue Thrusting
forward movement of the tongue pushing against teeth
Swallowing
tongue moves forward
protruson of head
Cervical Flexion
mandible goes anteriorly and superiorly
Cervical Extension
mandible goes posteriorly and inferiorly
Cervical Side Bending or Rotation
maximum occlusion on ipsilateral side
Functional Opening of the Jaw
place 2-3 PIPs joints or knuckles with mouth open
Endfeel
tissue stretch
MMT Grading
f- functional
wf- weak functional
nf- non-functional

lateral cervical spine to posterolateral skull
Occipitofrontal
above eye, over eyelid, and up over lateral aspect of skull
