What is tinea?
Fungal infection from DERMATOPHYTE FUNGUS (ringworm)
Tinia = dermatophyte
Candida = candida
Pityriasis versicolor = malassezia
Tinea given different names depending on where it affects:
tinea pedis = athletes foot
tinea corporis = affets limbs (aka ringworm)
How do you get tinea?
Usually caught from infected individual:
- shared towels, skin contact, barefoot changing rooms
Tinea pedis:
- occlussive footware
- excessive sweating / exercise
- diabetes
Use of steroids and immunosuppression.
Presentation of tinea?
Diagnosis?
- acute or gradual onsite
- usually itchy and inflammed (particularly feet)
- pedis affects spaces between toes, can be scaly
DIAGNOSIS
clinical
skin scraping can be sent (culture can take months but typical features might be seen on microscopy)
Differential Dx when considering tinea?
Tinea pedis:
- eczema
- psoriasis
- contact dermatitis
Tinea corporis:
- impetigo
- seborrheic dermatitis
- psoriasis
- eczema
- lichen simplex
- contact dermatitis
- pityriasis rosea
Management of tinea?
General:
- dry thoroughly
- avoid occlusive footwear
- use sandal in shared shower / changing
TOPICAL
- OD or BD
- terbinafine (lamsil)
- clotrimazole
- miconazole
ORAL
- resistant cases
- terbinafine 250mg for 2 weeks
- fluconazole 150mg once weekly for 6 weeks
- itraconazole
- treat shoes with anti-fungal powder
