What is the mechanism of carbimazole?
A
Stops the thyroid peroxidase enzyme from coupling and iodinating tyrosine

What are the side effects of carbimazole?
A
- Delayed onset
- Rash
- GI upset
- Agranulocytosis
- Treatment 1-2 years, relapse is 50%

What is the mechanism of propythiouracil?
A
- Binds to peroxidase and stops iodination of tyrosine

What are the side effects of propylthiouracil?
A
- Higher rate of agranulocytosis
- Inhibition of peripheral de-iodination
- Reserved for those intolerant of carbimazole

What is radioactive iodine?
A
- Complete destruction of the gland by beta radiation
- Decays with half-life of 8 days
- Requires subsequent T4 replacement

- Atrophic
- Primary myxoedema
- Hashimotos
- Drugs - lithium, amiodarone
