“FIRST”…
Indicates priority. May do all - but what do you do FIRST
Legionnaires’ Disease
A type of pneumonia caused by legionella bacteria.
Legionnaires’ disease doesn’t spread from person to person. Instead, the bacteria spreads through mist, such as from air-conditioning units for large buildings. Adults over the age of 50 and people with weak immune systems, chronic lung disease, or heavy tobacco use are most at risk.
Many people exposed to the bacteria don’t develop symptoms. Those who do develop symptoms may experience cough, fever, chills, shortness of breath, muscle aches, headaches, and diarrhea.
Legionnaire’s disease can be treated with antibiotics, such as azithromycin or ciprofloxacin.

Legionella is found in warm, stagnant water (such as hot water tanks) and is spread by mist from the environmental source to the client.
Captropril 25mg TID
Captopril is used to treat high blood pressure (hypertension), congestive heart failure, kidney problems caused by diabetes, and to improve survival after a heart attack. *ACE inhibitor.
Things to remember:
- Take 1 hour before meals, or 2 hours after
- AVOID foods high in potassium
- Do NOT use salt substitutes because they contain potassium
- Take medication at the same time everyday
- Change positions slowly to prevent orthostatic hypotension
Client presents to the ED with multiple wounds…
FIRST - ask the parents what caused the injuries. See if their explanations make sense
Symptoms of Tuberculosis (TB)
Dry cough for several weeks with frequent night sweats. Place patient in room with negative air pressure, and wear a mask until it is determined patient is free of disease.
Patient with RIGHT sided weakness
Client holds cane with his LEFT hand, moves the cane forward followed by his right leg and then his left leg.
Cane acts as support and aids in weight bearing for the weaker right side. Elbow should be flexed 30 degrees. Tip of cane should be 15cm lateral to the base of the 5th toe.
Rehydration after illness…
If dehydration is NOT relevant, child should reassume normal diet - want reintroduction of nutrients asap
Peripheral Vascular Disease (PVD)
A circulatory condition in which narrowed blood vessels reduce blood flow to the limbs.
Patients should sit with feet on the floor (or comparable surface) and avoid crossing legs or wearing restrictive clothing.
Sensitive/Sore breasts during pregnancy
Apply cool compress to breasts and wear a well fitting, supportive bra.
Ciprofloxacin
Antibiotic that treats infections. Clients on cipro should drink plenty of fluids! Do NOT take with milk or yogurt alone, because it decreases the absorption of cipro.
Colorectal Cancer
A cancer of the colon or rectum, located at the digestive tract’s lower end. Risk factors for colorectal cancer include high protein, high fat / low fiber diet.
The client wears a hearing aid in the left ear. Client is scheduled for a permanent pacemaker insertion.
Hearing aid battery may affect placement of pacemaker. Should not be placed under the left clavicle of this client.
Phenelzine Sulfate
Antidepressant - MAOI
Cushing Syndrome
A condition that occurs from exposure to high cortisol levels for a long time. Signs: (1) Fatty deposits around face & upper back (2) Weight gain around mid-section (3) Fat LOSS from arms and legs.
Treatment options include reducing steroid use, surgery, radiation, and medication.
CAN EAT: spinach and tuna fish sandwich. Both are high in potassium, and acceptable with MAOIs (most vegetables are).
NO BANNANAS: high in tyramine, which is contraindicated with MAOIs and can cause a hypertensive crisis.
After a total knee replacement
Apply a compression device to the patient’s lower extremities.
