The PeriOp Environment Flashcards
What 4 tasks are part of the PREoperative phase of periop care?
The tasks that are part of the PREop phase of care are:
- data collection
- pt assessment
- development of nursing dx
- development of plan of care (including outcome identification and planning)
What 4 tasks are part of the INTRAoperative phase of periop care?
The tasks that are part of the INTRAop phase of care are:
- implementation of the plan of care
- documentation of care provided
- coordination of intervention
- evaluation of nursing interventions (** occurs in both the INTRA and POSTop phases)
What 3 tasks are part of the POSToperative phase of periop care?
The tasks that are of the POSTop phase of care are:
- evaluation of nursing interventions that occurred (** occurs in both the INTRA and POSTop phases)
- identification of any complications
- communication of care given.
What are the 7 steps in the nursing process?
The steps of the nursing process are:
- assessment (data collection)
- diagnosis
- outcome identification (defining nursing goals and intervention strategies.)
- planning
- implementation
- evaluation
- documentation
Remember the acronym ADPIE (leaves out outcome identification)
The patient’s hair should be covered before entering which zone?
The patient’s hair should be covered before entering either the semi-restricted or restricted zones. The restricted zone includes the actual OR and any adjacent substerile areas.
Zones in the PeriOp environment:
The Un-restricted zone includes…
The Un-restricted zone includes…
- PACU (in some facilities)
- Endoscopy suite
- Pain clinic or procedure room
- Locker rooms
- Hospital corridors
- Depending on layout, may include PreOp, offices, waiting areas, lab, blood bank and pathology.
The Un-restricted zone includes entrances and exits. Street clothes are permitted. Temp should be 68-75 (20-24) with humidity between 20-60%, with 6-15 air exchanges/hr including 2-3 outdoor air exchanges.
Zones in the PeriOp environment:
The Semi-restricted zone includes…
The Semi-restricted zone includes…
- sterile processing
- equipment and sterile supply storage
- decontamination
- PACU (in some facilities)
The Semi-restricted zone requires head and body coverings. Temp should be 68-75 (20-24) with max humidity of 60%, with 4-10 air exchanges/hr including 2 outdoor exchanges. Traffic should be unidirectional if possible.
Zones in the PeriOp environment:
The Restricted zone includes…
The Restricted zone includes…
- OR
- Invasive procedure room
- any room within the sterile core
The restricted zone requires the same attire as the semi-restricted except masks are required if there are open sterile supplies. HVAC requirements are temp of 68-75 and humidity of 20-60%. Air should be under positive pressure with 20 air exchanges per hour including 4 outdoor exchanges. Air should be cleaned with a HEPA filtration system.
_____ gloves are worn to protect skin against ethylene oxide exposure in sterile processing rooms?
Thick, heavy duty gloves are worn to protect skin against ethylene oxide exposure in sterile processing rooms
Proper surgical attire consists of _________
Proper surgical attire consists of…
- scrubs (tucked in or close fitting)
- single use jumpsuits (for visitors in OR for brief period, eg pathologist, biomed)
- head cover (confining all hair, sideburns and neckline hair).
- surgical mask (should never dangle around neck)
- shoe covers (remove with protective gloves. Required in ortho and autopsies, recommended in C/S births and trauma)
- PPE
Basic surgical furniture found in the restricted zone includes…
Basic surgical furniture found in the restricted zone includes…
- mayo stand
- back table
- OR table
- ring stand
- kick bucket
Other equipment may include lights, monitors, anesthesia machine, blankets and compression devices. Emergency equipment such as defibrillators, crash carts and MH carts are not stored in procedure rooms. Sterilizers may be in the surgical suite but are not part of the basic furniture.
The PeriOperative Nursing Data Set or PNDS, an empirically validated tool using standard language, consists of a series of interventions that contribute to positive patient outcomes. These 4 domains represent fundamental knowledge and skills in the periop setting. What are the 4 domains (broad competency statements) of the PNDS?
The 4 domains of the PNDS include:
- safety – freedom from acquired physical injury.
- physiologic response - surgery goes as expected
- Behavioral responses – subdivided into Knowledge / Psychosocial and Rights/Ethics/Competency
- Health system - the environment of periop settings.
