What’s the definition of cardiac output?

A

Cardiac output =stroke volume x heart rate
 休息狀況下(成人):CO=72 beats/min×0.07L/beat = 5.0L/min  運動員運動下:CO=35L/min , 一般未訓練的人 CO=20-25L/min  心臟左右邊的CO是相同的。且一般HR和SV會呈反比

A

交感神經可刺激SA node以及整個心臟,增加其pacemaker potential的斜率。(藉由增加F-type通道的通透性,使閾值更快達到→心跳速率增加)。

副交感神經可刺激什麼?

A

副交感神經刺激可刺激SA node、atria、AV node,使pacemaker potential 的斜率下降,閾值更慢達到,心跳速率下降。另外,副交感神經可藉由增加K+通透性使SA node的細胞膜過極化(Hyperpolarizes),所以pacemaker potential 起始於更負的值(接近於鉀離子的平衡電位)。

如何改變end diastolic volume?

A

 The Frank-Starling Mechanism (又稱 Starling’s Law of the heart):因當肌肉伸張愈大,收縮力則愈大,所以當end-diastolic volume 增加,stroke volume 也會增加。
 意義:當Venous Return(靜脈回流心臟的血液)增加,會增加end-diastolic volume (stroke volume),使心臟輸出增加 (ex:當右心幫浦的血多於左心,血流入左心室的量也會增加,造成左心室輸出增加,上述之方式可確保血液不會累積於肺臟系統中)

如何改變sympathetic nervous system 輸入心室的強度?

A

 交感神經分布在整個心肌膜,其神經傳導物norepinephrine作用在beta-adrenergic receptors可增加Contractility (定義:在固定 end-diastolic volume 下的收縮強度)。
 除了增加收縮強度,也可使心跳速率增加(下右圖)。
 上述兩種與改變end-diastolic ventricular volume 無關,所以與 The Frank-Starling Mechanism 不同。
 下圖左:可得知交感神經使contractility上升,可造成更大的stroke volume
