How to assess and treat for a pneumothorax and tension pneumothorax

A

May occur with chest/abdomen/shouler/neck entry wounds

  • Significatnt chest
  • Severe respiratory distress
  • Decreased breat sounds on one side of chest
  • O2 less than 90%
  • Cardiac arrest w/o obvious fatal wounds

How to assess and care for a sucking chest wound

A
  1. Burb/remove chest seal
  2. Decompress injured side 5th intercostal space along anterior axillary line or 2nd ICS mid clavicular line (not inside the nipple
  3. If first successful but symptoms return, insert a second proximate to first with a new needle
  4. If unsuccessful insert a second needle at the different site

If CAS doesnt have pneumo. the needle wont make it worse

A

open pneumothorax

whole larger air enters the hole

apply vented seal over the wound at end of exhalation
burp and reseal to relieve pressure
if unsuccessful apply NCD
