Features of Testicular Torsion
Sudden onset pain in testicle. Uncomfortable to walk, Pain in abdomen, nausea and vomiting.
O/E one testicle may be higher than the other or lying horizontal and will be tender, hot, swollen.
Differential of testicular torsion
Epididymo-orchitis - tends to be in older patients and UTI symptoms.
Torsion of the hydatid of Morgagni - typically in young patients 7-12 - small blue nodule on testis removed.
Tests for testicular torsion
Doppler USS - blocked blood flow to testis.
Treatment for testicular torsion
Important to treat quickly 24hr then loss of testis is high.
Opening scrotum and fixation of testis bilaterally is typical.
If testis is non viable orchidectomy performed and fixation of remaining.
