What is testicular torsion?
A twist of the spermatic cord resulting in testicular ischaemia and necrosis.
What is the peak incidence age for testicular torsion?
Most common in males aged between 10 and 30, with peak incidence at 13-15 years.
What are the features of testicular torsion?
Pain is usually severe and of sudden onset; pain may be referred to the lower abdomen; nausea and vomiting may be present.
What is observed on examination of testicular torsion?
A swollen, tender testis retracted upwards; the skin may be reddened.
What reflex is lost in testicular torsion?
The cremasteric reflex is lost.
Does elevating the testis ease the pain in testicular torsion?
No, elevation of the testis does not ease the pain (Prehn’s sign).
What is the management for testicular torsion?
Treatment is with urgent surgical exploration.
What should be done if a torted testis is identified?
Both testis should be fixed as the condition of bell clapper testis is often bilateral.
