when do male get testicular cancer
Three peaks: infancy, 25-40 and 60
Solid testicular mass > 50 yoa= usually lymphoma.
RF for testicular cancer
Kleinfelter, cryptorchidosm, FHx, PHx
most common type of testicular cancer
GERM CELL tumor (exam). can further be divided into seminoma and nonseminoma.
Other types: sex cord tumor
Symptoms of testicular cancer
Painless testicular mass, can present with acute pain (bleeding into tumor), symptoms consistent with infectiois epididymitis
Dx of testicular cancer
USG
radical inguinal orciectomy minimizes local tumor occurance and aberrant lymphatic spread. Transscrotal orchiectomi = CI
Serum markers for testicular cancer
AFP (never increased in seminoma) HCG, LDH = exam
which LN are often infiltrated in testicular cancer
Paraortic (retroperitoneal)
metastasis in testicular cancer
Liver, lung, Bone
chemo therapy in testicular cancer
Cisplatin + etoposide +- bleomycin (nb lung fibrosis)
