What drug do you expect to give to a patient with an anterior wall MI?
beta blockers because they decrease the work of the heart and increase the threshold for ventricular fibrillation; the beta blocker of choice is metoprolol
What beta blocker should be avoided in patients with an anterior wall MI?
Propranolol because it is not a cardioselective beta blocker. It affects beta receptors in heart AND lung tissue. therefore, it is more likely to cause bronchoconstriction that a cardioselective beta blocker
What heart blocks can be noted in an LAD occlusion?
The LAD supplies the bundle of HIS, which should result in a second-degree, Type II heart block
What heart blocks can be noted in an RCA occlusion
The RCA supplies the SA and AV node.
What is Dressler’s syndrome?
a type of pericarditis that occurs as a result of an immune response after an MI, surgery, or traumatic injury
Complications of an RCA occlusion include
papillary muscle dysfunction or rupture of the mitral valve
IABP balloon inflation
increases coronary artery perfusion; inflates at dicrotic notch of the arterial waveform, beginning of diastole
IABP deflation
decreases afterload; deflates right before systole begins; determined by set trigger for deflation, R-wave of ECG or upstroke of arterial pressure wave
