Origin:
Long head: Infraglenoid tubercle on scapula
long head insert: Medial and lateral heads of olecranon process on the ulna.
Lateral head: Posterior surface of the humerus superior to the radial groove.
Medial head: Posterior surface of the humerus inferior to the radial groove.
Insertion: Common tendon insertion

Long head: Accessory adductor

Orig: Coracoid process of scapula
Insert: Middle 1/3 of MEDIAL surface of the humerus.

Flexion and adduction of arm.
Deltoid (whole muscle)
Origin:
Posterior: Inferior edge of the crest of the spine of the scapula
Middle: Lateral margin of the aromion
Anterior: Anterior border of lateral 1/3 of clavicle.
Insert: Deltoid tuberosity of humerus.
Axillary Nerve.
Abductor (after initial 15 degrees)
Anterior fibers flex arm
Posterior fibers extend arm.

Orig.: Superior nuchal line and Occipital Protuberand and Medial margin of ligamentum nuchae.
Insert: Superior edge of the crest of the spine of the scapula, acromion, posterior border of lateral 1/3 of clavicle

Elevates and rotates the scapula

Orig: Medial margin of ligamentum nuchae, Spinous processes of C7-?

Orig. Transverse process of C1 and C2 and Posterior tubercles of transverse processes of C3-C4
Insert. Posterior surface of medial border of scapula from superior angle to root of the spine of scapula.

Elevates scapula
Rhomboid Minor
Orig: Lower end of ligamentum nuchae. and Spinous processes of C7 and T1.
Insert: Posterior surface of medial border of scpula at the root of the spine of the scapula.
Dorsal Scapular Nerve
Elevates and retracts scapula.
Downward rotation of scapula.
Rhomboid Major
Orig: Spinous processes of T2-T5 and intervening supraspinous ligaments.
INsert: Posterior surface of medial border of scapula from root of the spine of the scapula to the inferior angle
Dorsal scapular nerve
Elevates and retracts scapula

Supraspinatus
Orig: Medial 2/3 of the supraspinous fossa
Insert: Superior facet on greater tubercle of humerus

Abduction

Sternal Head: Anterior surface of sternum.
Insert: Lateral lip of intertubercular sulcus of humerus.
