d. Metronidazole 500 mg IV plus ciprofloxacin 400 mg IV

A
  1. first bout of acute pancreatitis,
  2. biliary pancreatitis,
  3. patients needing frequent IV pain medication,
  4. not tolerating oral intake because of vomiting or increasing pain,
  5. persistent abnormal vital signs,
  6. signs of organ insufficiency (inc. creatinine, increased LFTs, lactate won’t go down w/ organ insufficiency)
