Present Level of Academic Achievement and Functional Performance
-helps determine child’s future goals and help them achieve them
Self-determination
Choosing and setting goals for self, being involved in life decisions, self-advocating, and working towards goals
-Children need to have self-determination in development of IEP to make it client focused
Indicator 13
State Performance Plan required by the Individuals with Disabilities Education Act (2004).
-measures “the percentage of youth aged 16 and above with an IEP that includes coordinated, measurable, annual goals and transition services that will reasonably enable the students to meet postsecondary goals.”
Transition services
Coordinated set of activities for a child with a disability
-Must include: Post-secondary education, vocational education, integrated employment, continuing education, independent living services
IEP must include
- Appropriate measurable post-secondary goals based on individual student (age, skills, assessments, etc.)
- Transition services need to assist child in reaching post-secondary goals
