toxic, febrile smoker w/ leukocytosis presents w/ deep seated pustules on the middle of plams and soles. They spread outward to involve the rest of the hand. The patients states that they happened when he was stressed out about his health change and new infection. DX:__ tx:__

patient w/ hx of being a dishwasher and automechanic. has damage to stratum corneum, inflamed, cracked, fissured skin. dx__ trx__

middle aged to elderly patient presents w/ complaint of a 1-5 cm round, coin shaped red plaque that returns to the sam spot every winter. The patient says the rash it MC on the back of the hand and they thought it was ring worm. DX__ trx___

patient presents w/ hx of moderate itch that preceded vesicles showing up on fingers, hand and feet. the vesicles resolved w/in 3-4 weeks then became itchy and scaley, then became painful. The vesicles were symetrical on the lateral aspects of the feet and fingers. The patient said they had just started working in the hospital and they were washing their hands more frequently. dx__ tx__
dyshidrotic eczema (pompholyx), avoid water, potent steroid then wean, abx
prurigo nodularis associated w/ lichen simplex chronicus

chronic, pruritic eczematous disease that usually begins in childhood. Typically associated w/ allergies, atopy, asthma, hot showers, humid environments. This usually begins as an itchy area that turns into a rash as a result of scratching. Commonly found on flexor areas, spares face except eyelids, and wrist and ankles. On an infant shows up at 3 months on cheeks. associated w/ ichthyosis vulgaris and keratosis pilaris. dx__ tre__
atopic dermatitis, emollients and antipuritics (hydroxyzine, diphenhydramine), tepid showers then apply emolient, consider abx. pimecrolimus cream/tacrolimus if >2 yrs old
old people present w/ dry skin resembling cracked porcelain on anterolateral legs following cold dry weather and long hot showers. DX__ Trx__
asteatotic eczema (eczema craquele) “winter itch”, emollients after showing, steroids
+ fam hx, worse in winter, Koebner phenomenon (trauma), red discrete flat-topped scaling papules that become thick, silvery white scaling plaques. If the scale is removed there is pinpoint bleeding called __ ___. Seen on __ and groin area, and __ surfaces. Treatment is: systemic if >_% of body, and topical if <_% of body. May need to use __ agent to remove scale prior to steroids. associated w/ nail ___. Dx: __

nummular eczema
Rash w/ fine white or yellow greasy flakes seen mostly in the hair line, eyebrows, base of eyelashes, and nasolabial folds. Referred to as __ __ in infants, and is usally brought on by dry winter, stress, and change in hygiene (field). Worse in elder neuro patients and common in AIDs patients. DX__ Trx__
cradle cap, seborrheic dermatitis, shampoos w/ antidandruff (selenium, selsun), if really bad think topical steroids for face, chest and back or ears. ketakonazole if yeast

young adult presents w/ hx of a single patch (2-10 cm) following viral URI. after first rash appeared the lesions began an eruptive phase and reached there max# in 1-2 weeks. The distribution looks like a christmas tree on trunk and proximal extremities and follows the longitudinal axis of the skin lines. DX__ trx__
pityriasis roasea, benign and self limited, acyclovir if HHV-6, and syphilis test
patient presents w/ complaint of rash and nodules that have presented as a result of habitual scratching. The patient states they derive great pleasure from scratching. It was precipitated by seborheic derm, atopic derm, nummular eczema, lichenification, or nerve entrapment. The patient has developed hyperpigmentation and nodules (prurigo nodularis) and plaques. You have seen this rash pretty much everywhere including scrotum and vulva. DX__ TX
lichen simplex chronicus, break itch cycle: 1st gen antihistamine, kenalog for nodules, class 1 steroids for thick skin
