Teratogens and Congenital Diseases Flashcards
ACE inhibitors
Renal damage
Alkylating agents
Absence of digits, multiple anomalies
Aminoglycosides
CN VIII tox (A mean guy hit the baby in the ear)
Carbamazepine
Facial dysmorphism, developmental delay, neural tube defects, phalanx/fingernail hypoplasia
DES
Vaginal clear cell adenocarcinoma, congenital Mullerian anomalies
Folate antagonists
Neural tube defects
Isotretinoin
Severe defects. Contraception mandatory
Lithium
Ebstein anomaly (atrialized right ventricle)
Methimazole
Aplasia cutis congenita
Phenytoin
Fetal hydantoin syndrome - cleft palate, cardiac defects, phalanx/fingernail hypoplasia
Tetracycline
Discolored teeth (teethracyclines)
Thalidomide
limb defects (phocomelia, micromelia - flipper limbs).
Valproate
Inhibition of maternal folate absorption -> neural tube defects.
ValproATE inhibits folATE absorption
Warfarin
Bone deformities, fetal hemorrhage, abortion, ophthalmologic abnormalities
Do not wage WARFARe on the babym keep it HEPpy with HEParin (does not cross placenta)
Alcohol
FAS. Pre- and postnatal developmental retardation, microcephaly, facial abnormalities (eg smooth philtrum, hypertelorism), limb discoloration, heart defects. Heart-lung fistulas and holoprosencephaly in most severe form. Mechanism of failure is CELL MIGRATION!
Cocaine
Abnormal fetal growth and fetal addiction. Placental abruption
Smoking (nicotine, CO)
Low birth weight (leading cause in developed countries), preterm labor, placental problems, IUGR, ADHD.
Iodine (lack or excess)
Congenital goiter or hypothyroidism (cretinism)
Maternal diabetes
Caudal regression syndrome (anal atresia to sirenomelia), congenital heart defects (eg transposition of great vessels), neural tube defects
Also, anencephaly
Also, CAUDAL REGRESSION SYNDROME - agenesis of sacrum to occassionally lumbar spine -> flaccid paralysis of legs, dorsiflexed contractures of feet, urinary incontinence.
Vitamin A (excess)
Extremely high risk for spontaneous abortions and birth defects (cleft palate, cardiac)
X-rays
Microcephaly, intellectual disability. Minimize with shielding
GI malformation associated with downs
duodenal atresia
Heart defects associated with turners
Coarctation of aorta and bicuspid aorta
Heart defects associated with turners
Coarctation of aorta and bicuspid aorta

Septal defects, PDA, pulmonary artery stenosis
Down Syndrome and heart
AV septal defect (ENDOCARDIAL CUSHION DEFECT), VSD, ASD
Infant diabetes mother and heart
Tranposition of great vessels
Marfan syndrome and heart

Ebstein anomaly
Turner syndrome and heart
Bicuspid aortic valve, coarctation of aorta
Williams syndrome and heart
Supravalvular aortic stenosis
22q11 and heart (DiGeorge)
Truncus arteriosus, ToF
Cretinism (congenital hypothyroid)
Pot-bellied, pale, puffy-faced child with protruding umbilicus, protuberant tongue, and poor brain development.
