- Mental tension

- Missing meals
- Fatigue
What are symptoms and signs of a tension headache?
- Dull
- Non-pulsatile
- Bilateral
- Constricting pain (not severe)
- Pericranial tenderness
- Normal neuro exam
- Frontal or occipital
What are possible DDx for tension headache?
- Chronic migraine
- Medicine overuse headache
- Sphenoid sinusistis
- Giant cell arteritis
- TMD
What investigations are used for tension headache?
clinical diagnosis
What is the management of tension headaches in an acute attack?
simple analgesia (paracetamol and NSAIDs)
What is the management of tension headaches with chronic symptoms (>7-9 headache days/month)?
1st line: antidepressants e.g. amitriptyline 10mg
2nd line: muscle relaxants: e.g. tizanidine
What are possible non-drug therapies for tension headache?
- Relaxation
- Electromyographic biofeedback
- CBT
- Physiotherapy
What are possible complications for tension headaches?
peptic ulcer from NSAID use
