Pathophysiology
A
- chronic tendon injury due to overuse- repetitive loading
- degeration, disorganisation of collagen fibres
- increased cellularity
- little inflammation

Role of the tendon
A
- transmits force from muscle to acheive movement
- parallel collagen fibres with tenocytes
- surrounded by paratendon sheath
- larglely avascular, nutrion via paratenin
muscle to bone!

Sites
A
- rotator cuff
- tennis elbow (lateral epicondylitis)
- Golfers elbow (medial epicondylitis)
- platar fascitis (not a tendon but affects same group)
- achilles

Clinical features
A
- functional pain
- tendon swollen thick and tender

Diagnosis
A
- xray unhelpful
- MRI good for unclear diagnsosi
- ultrasound is useful for loking at one specific tendon

Treatment
A
Non operative
- NSAIDS
- steroid injections impair own bodys attempt to repair
operative
- 50% of tendon debridement without loss of function
