What are the 4 phases of tendinopathy?
Stress shielding
Reactive tendon
Tendon disrepair
Degenerative tendon
What happens in stress shielding?
The tendon is exposed to load that it hasn’t previously been under
Why happens in reactive tendon phase?
Short term adaptation to thicken the tendon, no inflammation occurs
What happens at tendon disrepair?
Attempt at healing, which results in the collagen fibres separating/ disorganisation
What happens at degenerative tendon?
Tendon is beyond repair, lack of collagen and disorder to the tendon
What is the mechanism of tendinopathy?
Tendon stretches
Amino acids and bond breakage
Heat as a result of bond breakage
Heat doesn’t dissipate properly
Irreversible denaturing of matrix proteins
Tendon rupture.
What is the time scale of healing?
2-6months depending on severity, it’s also a continuum
