Female, 50-70
Fam history
PMR

Symptoms and signs
A
Throbbing headache, visual disturbance, jaw claudication, scalp/temple tenderness.
PMR: Myalgia, muscle weakness, joint stiffness.

Investigations
A
USS/biopsy temporum.
Bloods: FBC, UandE, CRP and ESR.
Anti-CCP and RF.

Management
Bisphosphonates side effects
A
Aspirin?
60mg prednisolone or IV methylpred.
2 year course, wean off.
Long term steroid use: Bisphosphonates, PPI, Vit D, calcium.
SE: Osteonecrosis jaw, GI disturbance, joint aches.
Take first thing in morning (drink), sat upright. No drinking for 30 minutes.

Blindness, seizures, strokes.
