- Person has redness, swelling, pain around IV or venous access site
- Person has a fever or other signs of infection

- Person is experiencing uterine contractions that are frequent or severe and are not relieved by resting
- Person has vaginal bleeding or there is fluid leaking from the vagina

A

- Change in vital signs especially an increase in pulse rate and decrease in blood pressure, or an increase in temperature

- Person who has epidural block complains of increased numbness in the legs and pelvic region or a headache
- Person having trouble urinating or is unable to urinate

- Baby cannot be wakened

A
- Client has signs of an infection like fever, pain,, redden skin, coughing, thick nasal discharge, vomiting or diarrhea
- Family member has signs of infection, especially if the client is at high risk for catching a communicable infection because of a weakened immune system
- Members of the household have poor sanitation practices
- House lacks running water or toilet facilities
- House is infested with insects or rodents
