When may a takeoff NOT be commenced?
When the RVR or cloud ceiling is below the minima for takeoff
If multiple RVRs are reported, how many need to be above the takeoff minima in order to depart?
All.
If no RVR is available, how may the visibility be assessed for takeoff?
By counting the runway lights.

If lower company or state minima is available where will this be shown?
Parentheses.
If takeoff is predicated on this then all RVR must be above the lower minima.
If takeoff minima is not available on the appropriate chart then:
Contact the appropriate authority to obtain the state minima.
If State minima cannot be obtained then what minima will apply?
If high intensity runway edge lights (HREDL) and runway centreline lights (RCLL) and runway centreline markings (RCL) are available:
-Zero ceiling, 200m RVR
If HREDL or RCLL or RCL are available AND the pilot has visual reference to the forward line of motion:
-Zero ceiling, 400m RVR
- *If either of the above cannot be satisfied then
- Zero ceiling, 800m RVR
When takeoff weather conditions at the departure airport, or other operational reasons exist that would preclude a return to land at the aerodrome of departure then:
Takeoff is only permitted if an alternate airport is available within 340nm.
Pre dispatch aircraft serviceability must be?
