Delegation
transferring to a competent individual the authority to perform a selected nursing task in a selected situation but the RN keeps the accountability
Supervision
the provision of guidance and direction, oversight, evaluation and follow up by the licensed nurse for accomplishment of a nursing task delegated to nursing assistive personnel
Prioritization
deciding which needs or problems require immediate action and which ones could be delayed until a later time because they are not urgent

Licensed Practical Nurses (LPNs) or Licensed Vocational Nurses (LVNs):
• Although practicing nurses know that LPNs often review a patient’s condition and perform data-gathering tasks such as observation and auscultation, RNs remain accountable for the total assessment of a patient, including synthesis and analysis of reported and reviewed information to lead care planning based on the nursing diagnosis
Examples of LPN
o Monitoring findings (as input to the RNs ongoing assessment)
o Reinforcing client teaching from a standard care plan)
o Performing tracheostomy care
o Suctioning
o Checking NG tube patency
o Administering enteral feedings
o Inserting a urinary catheter
o Administering medication (excluding IV medications in some states)

Five Rights of delegations: Right task
• A right task is repetitive, requires little supervision, and is relatively noninvasive for a certain client
Five Rights of delegations: Right Circumstance
• Assess the health status and complexity of care the client requires, match the complexity of care demands to the skill level of the team member, and consider the workload of the team member

• Continually review the performance and determine care competence. When necessary take steps to remediate any failure to meet standards

Five Rights of delegations: Right Supervision
• The RN remains accountable for the total care of the patient throughout the tour of duty
o Occurs after delegation and determines whether completion of tasks is on schedule, performance was at a satisfactory level, delegatee documented and reported unexpected findings, delegatee needed assistance to complete assigned task in a timely manner, supervising nurse should reevaluate and possibly change the assignment
