  • immune complex deposition can affect organs

Features of SLE?

A
  • fatigue
  • fever
  • mouth ulcers
  • lymphadenopathy

-malar butterfly rash (spares nasolabial folds)
- discoid rash
- photosensitivity
- Raynaud’s phenomenon
- lived reticularis
- non scarring alopecia

  • arthralgia
  • non erosive arthritis
  • pericarditis (most common)
  • myocarditis
  • pleurisy
  • fibrosing alveolitis
  • anxiety and depression
  • psychosis
  • seizures

  • 99% are ANA positive
  • anti-dsDNA
  • anti-Smith

ESR and anti-dsDNA is used for disease monitoring

Management for lupus nephritis

A
  • treat HTN
  • glucocorticoids with either mycophenolate or cyclophosphamide
  • mycophenolate is preferred to azathioprine to decrease end stage renal disease
