A
decreased FEV1/FVC less than 40% TLC > 100% RV > 150% no hypoxia no hypercarbia acceptable cardiac risk abstinence from nicotine for 6 months

A
redo chest paCO2 > 60 on RA PA HTN ILD Vent dependence Prednisone > 20 mg /day just looks bad

VENT trial shows efficacy but more frequent infections and hemoptysis
