A product quality deficiency which may cause; death, injury or servere occupational illness; would cause loss of or major damage to weapon system; direcly restricts the combat readness capabilties of a using org; or which would result in a production line stop page.
What are the two types of supply discrepancy report
Shipping and packing
Shipping
These discrepancies deal with items shipped. They include shortage,overage,substitute,or incorrect item shipped
Packing
These discrepancies deal with the material the items were shipped in. They can include any condition caused by improper packing: damage ,loss ,or deterioration
SDR
The SF-364 form must be used to report
Repeated discrepancies involving items valued at $5 per line item
Discrepancies in shipments of classified material should be report only of the dollar amount per line items execs…

Who is responsible for completing and turning in SDRS and QDRS to the appropriate ICP
It is the RCVG units responsibility to complete and submit SDR for packing and shipping discrepancies to the appropriate activity. The SDR allows the appropriate activity to take actions to correct the deficiency. The RCVG unit is also responsible for completing and submitting QDRS when deficient items are RCVED and can not be used

Report crated when the quality of an item does not meet standards and must be returned

Report crated when a discrepancy is found in a received shipment formerly known as a report of discrepancy

How many copies of the completed SF-368 (QDR) must be submitted
The SF-368 is always submitted in triplicate, with one copy for unit records
Retain file copies of QDR forms for toe years after completion action
Depot-level repairable (DLR)
Depot-level reparable are high-cost equipment or parts that can be serviced a central site and then made available for future operations. DLR items management pertains to DOD, and MTR to Coast Guard -owned equipment. MTR is the term uses in CMPLUS
Mandatory turn-in repairable (MTR)
Reparable Coast Guard - owned equipment is designed by MTR. This is also the tee missed in the CMPLUS for DLR items
