What is a SCR
A
‘Read only’ copy of the GP patient record. Written consent not needed but verbal consent is required. It contains allergies and adverse reactions, all medicines and additional info such as diagnosis, immunisations etc.

Why might it be incomplete?
A
Home care medications may not have been delivered directly to patient, hospital discharge summary may not be updated by GP practice, patient is taking part in a clinical trial, handwritten Rx following a home visit which has not been added to GP practice
