Respiratory examination

A

In summary, I performed a respiratory exam on … , they had no peripheral stigmata of respiratory disease, their pulse was regular and they had a normal respiratory rate, they had a normal and symmetrical percussion note on the front and back of the chest. On auscultation they had normal vasicular breathing with no added sounds. This was a normal respiratory exam.

To complete the exam I would like to assess peak flow, measure their blood pressure, oxygen saturation and also send off a sputum sample.

Cardiac examination

A

In summary, I performed a cardiovascular exam on … , on inspection he had no peripheral stigmata of cardiovascular disease, his pulse was regular. On Palpation he had an undisplaced apex beat and he had no heaves or thrills. On auscultation he had normal first and second heart sounds with no added sounds. This was a normal cardiovascular exam.

To complete I would like to check the femoral pulse and synchrony with the radial pulse, check the blood pressure in both arms and lying and standing in one arm, perform ophthalmoscopy and obtain a 12 lead ECG.

Abdominal examination

A

In summary, I performed an abdominal exam on…, they had no peripheral stigmata of gastrointestinal disease, their abdomen was soft and non tender, with no masses or organomegally, on auscultation he had normal bowel sounds. This was a normal abdominal exam.

To complete exam, I would like to examine the inguinal lymph nodes, the stools if clinically indicated, the hernial orifices, do a rectal examination, obtain urinalysis, and examine the genitalia.

Upper Limb

A

In summary I performed an upper limb examination on… , On inspection there was no scars, wasting, fasiculations, involuntary movements or tremors noted. The tone, power and reflexes were normal throughout and coordination was intact. Sensation was normal across all dermatomes. In summary this is a normal neurological upper limb examination.

To complete this examination I would perform a full exam on the cranial nerves, lower limbs and I would assess the gait.

Lower limb

A

In summary I performed a lower limb examination on… , On inspection there was no scars, wasting, fasiculations, involuntary movements or tremors noted. The tone, power and reflexes were normal throughout and coordination was intact. Sensation was normal across all dermatomes. In summary this is a normal neurological lower limb examination.

To complete this examination I would perform a full exam on the cranial nerves, upper limbs and I would assess the gait.
