Auscultation
Demonstrate each osculatory point on anterior and posterior chest
VERBALIZE-
1) using the diaphragm of stethoscope placed firmly on uncovered skin
2) comparing side-to-side
3) instructing the client to breathe slightly slower and deeper than normal, with the mouth open
4) listening to one full inspiration and expiration in each location
5) avoiding breast bone and tissue, placing the stethoscope in the intercostal spaces

TEACH THE PATIENT
Reassure the patient that “if you have any difficult breathing, I will discontinue the suctioning.”
Adjust suction
100-150 Hg
clean glove, occlude the end
Place tubing
in convenient location
Remove gloves
Increase the patient’s
supplemental oxygen
Prepare
Towel on chest
Put on goggles
Open sterile packaging
OPEN AND POUR WATER BEFORE DONING STERILE GLOVES
With dominant hand
pick up sterile catheter
With non dominant hand
pick up connecting tubing and connect
Moisten
the catheter by dipping into sterile water and occlude the Y-tube to check suction
Remove oxygen
with your pinky, encourage patient to take several deep breaths
Insert the catheter
5 to 6 inches to reach the pharynx. Only suctioning 10-15 seconds at a time.
Offer
oral suction. Advancing the catheter 3 to 4 inches
Replace
oxygen delivery device
Flush
the catheter
Dispose of gloves, catheter, sterile saline
perform hand hygiene, remove goggles, raise the bed rail and lower bed
Assist
patient into comfortable position

oral hygiene

Evaluate the patients response to procedure. Reassess respiratory status, including respiratory rate, effort, oxygen saturation, and lung sounds.
