Q

Anders is a medical student and is in a clinical placement where he has the opportunity to take a patients blood, under the supervision of a junior doctor. He has a clinical examination tomorrow and has learnt the technique of taking bloods but still lacks confidence.

How appropriate is the following:
(1) ask the junior doctor to supervise hims whilst taking the bloods of other patients who require it
(2) review lecture notes on the blood taking technique

A

(1) ask the junior doctor to supervise hims whilst taking the bloods of other patients who require it - VERY APPROPRIATE - he’s lacking confidence and additional knowledge that would he gain from additional practice, and a junior doctor is supervising.

(2) review lecture notes on the blood taking technique - APPROPRIATE BUT NOT IDEAL - if Anders is lacking confidence he has likely already reviewed his lecture notes containing the information of the blood taking technique and so hands on practice is the best way for him to gain confidence in this area and develop clinically

Mia is a medical student at an antenatal clinic in a rural community that is understaffed and under resourced, it only has 1 doctor and 3 midwives , all but one midwife is busy and a patient comes in and is having a baby. The midwife is experienced in delivering babies and requests Mia’s assistance in delivering the baby but she is unsure on how to help and is uncomfortable.

How important is the following:
(1) the midwife is experienced in delivering babies

A

(1) the midwife is experienced in delivering babies - IMPORTANT - because Mia is not and is very obviously lacking in terms of knowledge that is relevant to the situation and so is required to work under supervision and direction of an experienced and senior clinician

Q

Medical student, Jack, is currently on placement in a surgical department when a registrar has asked him to suture a large cut on a patients leg. Jack is unsure on how to suture a wound and is uncomfortable doing so.

How appropriate are the following:
(1) “it is not appropriate for you to ask students to do this”

A

(1) “it is not appropriate for you to ask students to do this” - THIS IS INAPPROPRIATE BUT NOT AWFUL - as whilst the abilities of students vary and this response allows the registrar to know that jack is lacking in knowledge to undertake this procedure it is unnecessarily rude and is not constructive

Q

A medical student is due to undertake a procedure in front of a doctor and multiple medical students - the medical student gets a call from their mother stating that their grandmother is seriously ill. The medical student is emotionally distressed and would like to comfort her mother.

How important is the following:
(1) the impact of the students behaviour on her assessment grade
(2) the welfare of the students grandmother

A

(1) the impact of the students behaviour on her assessment grade - NOT IMPORTANT AT ALL - as the patients interest is of the greatest importance every time in medicine and so the medical students grade is not important, if the student is too distressed to carry out the procedure then they should forfeit this in order to protect the patients interests.

(2) the welfare of the students grandmother - this is important as it may impact the ability of the medical student to carry out the procedure

The receptionist complains to him about this.

How appropriate is the following:
(1) Apologise to the receptionist and say that he will make an effort to improve

A

(1) Apologise to the receptionist and say that he will make an effort to improve - THIS IS VERY APPROPRIATE- as it shows that Justin has taken what the receptionist has said to heart and he will try to improve - this is a necessary aspect of a clinical team

(2) that she has never conducted the procedure using this tool before

A

(1) that the senior dentist may not be aware of what tool Serena has been trained on at university - IMPORTANT - as both the dentist and Serena are responsible for ensuring patient care and so it’s important that Serena discusses her limitations with the senior dentist

(2) that she has never conducted the procedure using this tool before - VERY IMPORTANT - both the dentist and Serena are responsible for ensure the maintenance of patient care and that patient care is not compromised; it is very important that her lack of expertise are communicated to the senior dentist
