Stroke, Seizure, Eyes (Spark Notes) Flashcards
Alteplase Pre-Nursing Assessment
- Check current meds for anticoags and antiplatelets
- What is the onset of the stroke symptoms? Don’t give if past 4 hours or if hemorrhagic stroke
- Do they have conditions that may worsen by the dissolution of clots?
- Neuro Assessment
- VS
Brinzolamide AE
A pressure regulator eye drop
AE: Stinging, burning, eye discomfort
Client Education on Phenytoin
- Labs need to be taken for therapeutic levels (10-20)
- Don’t skip or stop suddenly
- Be careful in those who are old or debilitated or have renal/hepatic issues
- Watch for signs of toxicity (severe liver toxicity, bone marrow suppression, gingival hyperplasia, potentially serious dermatological reactions, frank malignant lymphoma)
- Avoid alcohol and other CNS depressants
- May last longer than expected
- Is teratogenic
Therapeutic effects of carbamazepine
Treats generalized and focal seizures
Thought to inhibit polysynaptic responses and block sodium channels
Lamotrigine (seizure med) client education
- Can cause CNS effects, mild and life-threatening rashes, aseptic meningitis
Topiramate AE
CNS depression, metabolic acidosis, angle closure glaucoma, suicidal thoughts and behaviors
Diazepam Indications
For seizures (a benzodiazepines)
May also potentiate effects of GABA
Causes muscle relaxation and relieve anxiety
Lab studies prior to starting antiepileptic meds
- CBC
- Liver/ Kidney functions
- Vitamin D levels
- Vitamin K levels
- Pregnancy Test
- Serum tests
Barbiturates nursing assessments
- Renal and hepatic testing
- Prego test
- Coma, depression, and psychoses assessment
- Warfarin use
- Vit K and D levels
- Any physical dependency or withdrawal?
Barbiturate TA
Inhibits impulse conduction in ascending RAS, depresses cerebral cortex, alters cerebellar function, and depresses motor nerve output
Alteplase TA
Works to activate natural anticlotting system and activates tissue plasminogen
Alteplase AE
MENTAL STATUS CHANGES
- Bleeding
- Cardiac arrhythmias
- Hypotension
- GI bleeding
- Hypersensitivity
Timolol eye drop use
Beta blocker, used to decrease the production of intraocular fluid (pressure regulator)
Loteprednol and Prednisolone uses
Steroid eye drops for intraocular inflammation
Brinzolamide and Timolol uses
Pressure regulator eye drops, for decreasing the production of intraocular fluid
Latanoprost Client Education
Can cause eyes to darken, dark eyelids, long eyelashes, eyelid droop, sunken eyes, stinging, burning, and redness
Latanoprost Uses
Pressure regulator eye drop, to increase aqueous outflow in open angle glaucoma
Erythromycin and Bacitracin uses
Antibiotic eye drops, for superficial bacterial ocular infections
Moxifloxacin Uses
Antibacterial eye drops, for corneal ulcers from contact lenses
Azithromycin uses
Antibiotic eye drops, for bacterial conjunctivitis
Tobramycin uses
Antibiotic eye drops, for external ocular infections
Alteplase Post nursing assessments
- Assess for side effects like MENTAL STATUS CHANGES, bleeding, GI bleeds, hypotension, cardiac arrhythmias, and hypersensitivity reactions
