Headaches, Alzhimers, Parkinsons Flashcards
Ergot Derivatives Prototype
Dihydroergotamine
Ergot Derivatives TA
Constrict cranial vessels, decrease cranial artery pulsation, decrease hyperperfusion of basilar artery bed
Ergot Derivatives Indications
Prevent or abort migraine or vascular headaches
Ergot Derivatives Pharmaco
Rapid absorption
Onset 15-30 mins
Hepatic
Bile excretion
Ergot Derivatives Contraindications
Known allergy
CAD, hypertension, PVD
Impaired liver function
Prego + Lactation
Ergot Derivatives Cautions
Pruritus
Ergot Derivatives AE
EGROTISM!!!
N/V, lightheaded, numb/tingling, hyper/hypotension, bradycardia, muscle pain + itching, chest pain
Ergot Derivatives DD Ints
Beta-blockers
Triptans
Triptans Prototype
Sumatriptan
Triptans TA
Cause vasoconstriction of cranial vessels
Binds to selective serotonin receptor sites
Triptans Indications
Treats (NOT PREVENTS) acute migraine
Also used for cluster headaches in adults
Triptans Pharmaco
Rapid absorption
Hepatic
Urine excretion
Cross placenta and milk
Triptans Contraindications
Known allergy
Pregnancy
CAD
Triptans Cautions
Older adults
Pts with CAD risk factors
Lactation
Renal/hepatic dysfunction
Triptans AE
Dizziness, drowsiness, weakness
Nausea
Flushing, sweating, tingling/numbness
Neck pain
Almotriptan reported to have fewer side effects than other triptans
Triptans DD Interactions
Ergot containing drugs
Triptans Routes and times
Sumatriptan: SQ, Intranasal, Oral (15 mins, 20 mins, 90 mins)
Naratriptan: Oral (1-3 hrs)
Rizatriptan: Oral (.5-2 hrs)
Zolmitriptan: Oral (1hr)
Almotriptan: Oral (1-3 hrs)
Frovatriptan: Oral (2-4 hrs)
Indirect-Acting Cholinergic Agonists Prototypes
Donepezil and Rivastigmine
Indirect-Acting Cholinergic Agonists TA
Slows down, not cures
Blocks acetylcholinesterase at the synaptic cleft and allows acetylcholine accumulation
Indirect-Acting Cholinergic Agonists Indications
ALZHEIMERS
Myasthenia gravis, glaucoma, treatment after nerve gas exposure
Indirect-Acting Cholinergic Agonists Pharmaco
Well absorbed
Some are metabolized by liver enzymes others primarily unchanged
Eliminated via kidneys
Indirect-Acting Cholinergic Agonists Contraindications
Bradycardia
Intestinal or urinary tract obstruction
Indirect-Acting Cholinergic Agonists Cautions
ANY CONDITION THAT COULD BE EXACERBATED BY CHOLINERGIC STIMULATION (asthma, urinary/GI obstruction)
Hepatic/renal dysfunction
Prego + lactation
Indirect-Acting Cholinergic Agonists AE
CHOLINERGIC CRISIS: Dumbells (Diarrhea, urination, miosis, bradycardia, emesis, lacrimation, lethargy, salivation)
