4. Patient was ambulatory at baseline (prior to event) Yes No Unknown
5. CBG between 60 & 400 Yes No
If questions 1 – 5 are all answered “Yes” or “Unknown” and at least 1 of the 4 neurological
examination findings are abnormal the patient is considered to have a POSITIVE screen.
