What are the different types of vaginal discharges and conditions that cause it?
Smells fishy - bacterial vaginosis
Thick and white, like cottage cheese - thrush
Green, yellow or frothy - trichomoniasis
With pelvic pain or bleeding - chlamydia/gonorrhoea
With blisters or sores - genital herpes
What is herpes labialis and causative organism?
viral infection of mouth area/lips, due to HSV-1 causing blisters/sores
What is genital herpes and causative organism?
viral infection of genital area due to HSV-2 causing blisters/sores
What is Mx for genital herpes?
Acyclovir
What is syphilis caused by?
Caused by bacteria called Treponema pallidum (spiral shaped, spirochete).
How can syphilis be transmitted?
Oral, vaginal, anal sex, vertical transmission (mother to baby), IV drug use.
How can syphilis present?
- Painless genital ulcer (chancre)
- Local lymphadenopathy
How to Mx syphilis?
- Refer to GUM clinic
- Advice about avoiding sexual activity until treated
- contact tracing
Medical Mx
- deep intramuscular dose of benzathine benzylpenicillin
What is chlamydia caused by?
Caused by Chlamydia trachomatis, a gram-negative bacteria.
What is the diagnosis of chlamydia?
Nucleic acid amplification tests (NAAT) are used to diagnose chlamydia.
What is the Mx for chlamydia?
Refer to gum clinical
1st line Medical treatment: doxycycline
What is gonorrhoea caused by?
Caused by Neisseria gonorrhoeae (gram-negative).
What is presentation of gonorrhoea?
- Odourless purulent discharge, possibly green or yellow
- Dysuria
- Pelvic pain in women
- esticular pain or swelling (epididymo-orchitis) in men
What is used for diagnosis of gonorrhoea?
Nucleic acid amplification testing (NATT) is use to detect the RNA or DNA of gonorrhoea
Standard swab for microscopy, culture and antibiotic sensitivities.
What is the Mx for gonorrhoea?
Refer to Gum clinic
- Ceftriaxone (if sensitivities are not known)
- Ciprofloxacin (if antibiotic sensitivities are known);
