How to diagnose hypertension?
If patient comes in with clinic BP >140/90mmHg send for ABPM or HBPM.
What is stage 1 hypertension?
-ABPM daytime average or HBPM average BP >= 135/85 mmHg
What is stage 2 hypertension?
-ABPM daytime average or HBPM average BP >= 150/95 mmHg
What is severe hypertension?
Clinic BP >180mmHg systolic or >110mmHg diastolic
What is the Mx BP <135/85mmHg
Monitor
What is the Mx for Stage 1 hypertension?
If <80 years old with one or more:
- target organ damage, -established cardiovascular disease
- renal disease
- diabetes
- 10 year cardiovascular risk of 10% or more.
Begin antihypertensives.
What is the Mx for stage 1 hypertension for 87 year old with clinic bp of 145/85?
Monitor and lifestyle advice
What is the lifestyle advice for hypertension?
- reduce salt intake ideally <6g/day
- Reduce caffeine intake
- weight loss
- less alcohol
- stop smoking
What is the Mx for stage 2 hypertension?
Start antihypertensive regardless of age.
