AStandard CareArrangement (SCA) is a writtenagreementbetween a Certified Nurse Practitioners (CNP), Certified Clinical Nurse Specialists (CNS) or Certified Nurse-Midwives (CNM) and a collaborating physician that formally establishes a collaborative practice arrangement.

These arrangements and agreements are state specific
Important to know the requirements in the state in which you plan to practice
Depending on the size of your practice site – this form may be prepared for you
In a small private practice, you may need to prepare your own agreement for signature(s)
Read and know your agreement before signing
You must have a valid and signed agreement before beginning to practice
It is to be kept on file at your place of employment - you should keep a signed copy for yourself
You must notify the Board of Nursing when you change a collaborating physician(s) and/or practice site and then complete a new SCA
Know & Follow the State’s Board of Nursing guidelines in the state in which you plan to practice
