skin appears red and does not turn back to normal color
reddened area can become pale, white or shiny

Stage 2 pressure ulcer
A
Blistering, an abrasion, or shallow crater epidermis (outer layer of skin) peels away or cracks open, creating a portal of entry for pathogens the dermis (middle layer) may be partially worn away

A
The epidermis (outer layer of skin) and dermis (inner middle layer)are gone and the subcutaneous fat may be visible in crater
