Palpate - face back wall

use three fingers, one on left of spinal column, one in middle and one on right. start at neck

  • examine above clavicle for masses
  • palpate the sacroiliac joints

Feet 30 cm apart
with your right arm reach down on your left leg as far as possible
and do the same on the other side
AND MEASURE HOW FAR OFF THE GROUND (<10 cm is normal)

SCHOBERS TEST (Measure and pen)

Use your thumbs to feel the posterior iliac spines (two sticky out things at lower back) then mark an x with a pen in between them on the spine. measure 10cm above and 5 cm below.
Get them to flex and measure again - it shouldn’t increase by more than 5cm

Lift leg up to 45 and then at top bend foot towards pt

Femoral stretch test

“turn over on to front for me. I’m now going to lift each leg, do let me know if you have any pain at all”

Hold ankle and flex knee to 90 and then put hand on to bottom of thigh and lift the leg up

On palpation no tenderness of spine or paraspinal muscles.
He had a full range of movement.
This was a normal spinal exam

BACK PAIN = ABDO, RECTAL, HIP EXAM

A

note:
someone with lower back pain, perform an abdominal examination and rectal exam (masses, cauda equina syndrome)
In patients presenting with neck pain, you should also examine the shoulder joints.
