Soft tissue lesions and biopsy techniques Flashcards
What is a biopsy
Sample of tissue for histopathological analysis to confirm or establish a diagnosis
What type of biopsies are there
Aspiration
Fine needle aspiration
Excisional biopsy
Incisional biopsy
Punch biopsy
When would you do a excisional biopsy
Involves Removal of all
clinically abnormal
tissue
Usually benign lesions
e.g. fibrous
overgrowths, denture
hyperplasia,
mucocoeles
When would you do a Fine needle aspiration biopsy
Aspiration of cells from solid lesions e.g.Neck swellings, salivary gland lesions
Cytology
When would you do a Incisional biopsy
Involves a representative tissue sample of Larger/uncertain lesions
E.g. Leukoplakia, lichen planus, squamous cell
carcinoma
Whats areas should you avoid when taking a biopsy
salivary gland duct orifices, tip of tongue, areas close to nerves and larger blood
vessels
name 5 soft tissue lesions
(Any of)
-Carcinoma
-Denture hyperplasia
-Fibrous epulis
-Fibrous overgrowth
-Giant cell epulis
-Pregnancy epulis
-Haemangioma/lymphangioma
-Lipoma
-Pyogenic granuloma
-Squamous cell papilloma
-Salivary gland lesions
-Lichen planus
-Lichenoid reactions
-Pemphigus
-Pemphigoid
-Behcet’s
-Leukoplakia
-Erythroplakia
What is a fibrous epulis
Swelling arising from the gingivae
Hyperplastic response to irritation e.g. Overhanging restoration, calculus
Smooth surface, rounded swelling, pink and pedunculated Removed with a Excisional biopsy
Remove source of irritation
What is giant cell epulis
Peripheral giant cell granuloma
Multi nucleated giant cells in vascular stroma that is coloured a Deep red or purple, broad base
Need x-ray to ensure not centrally originating )
Surgical excision
What is a haemangioma
A Hamartomata Developmental overgrowths that is blue in colour
Pressure will cause loss
of colour and Surgical removal or more commonly cryotherapy
What is a lipoma
A benign neoplasm of fat that Soft swelling, Pale yellow and Sessile
Excision to remove
What is a Pyogenic granuloma
It Arises from failure of normal healing and is a overgrowth of granulation tissue
May be related to extraction sockets or traumatic soft tissue injuries
Its Red in colour and requires Surgical excision
What is a Squamous cell papilloma
It is a benign neoplasm that is usually pedunculated
Has a white surface that looks like cauliflower
Excision at base needed
What is leaf fibroma
Chronic irritation
from denture that would be round if not covered by
denture but becomes flattened
Needs excision
What is a mucocoele
Mucus extravasation
cyst
Most common minor
salivary gland problem where Saliva leaks into
submucosal layer
It is recurrent
