Effects of alpha 2 agonists?
A
- suppression of norepi release by neg feedback
- transient HYPERtension followed by sustained hypotension
- vasoconstriction of arteries and veins (vascular smooth muscle)
- decreased gut motility
- inhibition of lipolysis
- sedation
- analgesia
- inhibition of INSULIN release from pancreas
- platelet aggregation

advantages of dexmedetomidine - 5
A
- powerful sedative
- powerful analgesia (200x potency of xylazine)
- good muscle relaxation
- significantly decreases requirement for other general anesthetics by 30-60% (dose dependent)
- minimal resp depression

disadvantages of dexmedetomidine -
A
- peripheral vasoconstriction
- profound bradycardia
- decreased CO
- do NOT use in preexisting CV Dz pts
- don’t use anticholinergics for bradycardia - increasing heart rate in the face of vasoconstriction and hypertension may overwork the heart, without actually improving cardiac output, and can cause malignant arrhythmias
