Enuresis
Bed wetting
Not a disorder until child > 5 years old

Treat initially with behavioral therapy (bed alarms)

Can use a TCA (imipramine) in refractory cases

Occur 3 or more times a week for AT LEAST 3 months
Insomnia
Excessive daytime sleepiness and daytime sleep attacks with decreased REM sleep latency. Occurs at least 3 times a month for 3 or more months
Narcolepsy.
Can see decreased levels of hypocretin in CSF
Narcolepsy
Treatment for narcolepsy
Amphetamines (methylphenidate) or nonamphetamine stimulants (modafinil)
Problem between when patient would like to sleep and when they actually do. Secondary to jet lag or shift work
Circadian rhythm sleep-wake disorder
Non-REM sleep arousal disorders
Sleepwalking and sleep terrors
Cannot be comforted be others and cannot remember the dreams
Sleep terrors
REM sleep disorders
Nightmare disorders, REM sleep behavior disorder, and restless legs
Frightening, well-remembered dreams. Greater risk for suicide.
Nightmare disorder
If nightmare disorder is comorbid with PTSD treat with
Alpha blockers
Arousal from sleep with vocalizations and running, kicking, punching. Usually seen with narcolepsy
