Diagnosis requires either depressed mood OR anhedonia (loss of interest/pleasure) + 5 SIGECAPS
MDD
Has fewer symptoms. Occurs within 3 months of a stressor and lasts less than 6 months
Adjustment disorder with depressed mood
Common sign of major depressive disorder in the elderly
Cognitive decline (pseudodementia)
If severe MDD presents with psychotic symptoms
Temporarily treat with antipsychotic in addition to antidepressant
Continue treating depression for ___________ months at an effective dose
6-12
Best for refractory or catatonic depression
ECT
Also used for acute mania or acute psychosis, or patients who refuse to eat or drink
ECT
Pregnancy is not a contraindication to
ECT
Intracranial mass, aneurysm, recent MI, recent stroke contraindications to
ECT
1 week of abnormally and persistently elevated, euphoric, expansive, or irritable mood
Manic episode
Must meet criteria for both manic and depressive episodes for 1 week or more
Mixed episode
No marked functional impairment or psychosis. No hospitalization required. Symptoms last for 4 days or less.
Hypomania
Chronic cycles of mild depression (dysthymia) and hypomania for 2 or more years
Cyclothymia
At least 1 hypomaniac episode alternating with at least 1 major depressive episode
