Classifications of Narcolepsy
Narcolepsy type 1 (WITH cataplexy)
Narcolepsy type 2 (WITHOUT cataplexy)
What hormone is deficient in T1 Narcolepsy?
Orexin
Proportion of NREM and REM sleep
NREM - 80%
REM - 20%
OSA Definition / Polysomnography findings
Upper airway narrowing/closure w/ ongoing respiratory effort
On polysomnography -> apnoea followed by respiratory effort
CSA Definition / Polysomnography findings
Reduction/cessation of airflow due to absent/reduced respiratory effort
On polysomnography -> Apnoea not followed by respiratory effort
CSA Mx
Treat underlying pathology - optimise HF therapy
1st line therapy - CPAP, O2 if hypoxaemic while sleeping
2nd line therapy - Adaptive seroventilation (contraindicated if EF <40%)
Narcolepsy Mx
Stimulant therapy - dexamphetamine
SSRI/SNRI for cataplexy
CSF findings in T1 Narcolepsy
Low hypocretin levels
Insomnia Mx
